PORTICO Community Church is a Christian church, incorporated as a registered Canadian not-for-profit charity, affiliated with the Pentecostal Assemblies of Canada (PAOC), serving the Greater Toronto Area (GTA) with campuses located in Mississauga and Milton.

PORTICO's Senior Pastor, elected by the official members, reports to the Board of Elders on vision, direction, ministry and church operations. The Senior Pastor is empowered and monitored through policy governance for the hiring, management and care of all ministry staff and for the oversight of ministries, facilities and operational matters.

PORTICO CARE CENTRE INTAKE COORDINATOR

Under the supervision of the Associate Pastor, SERVE, the PORTICO Care Centre Intake Coordinator, will be the first point of contact for individuals seeking care at PORTICO Care Centre. This role involves assessing and discerning client needs, providing information about available care options, and coordinating referrals to PORTICO Care Centre’s mental health, financial health, physical health, and spiritual health care providers and programs. The PORTICO Care Centre Intake Coordinator is empathetic, organized, and passionate about connecting people with the resources they need to thrive.

This role necessitates a leadership style that is servant-hearted and characterized by Christ-like attitudes. The leader must embrace a passion for individual spiritual development, exercise open and honest communication, be intentional in committing to initiating reconciliation & restoration when in disagreements, and express and maintain loyalty, confidentiality & support of the church leadership. Strong technical and operational skills and experience, as well as a thorough understanding and commitment to working in a large church, multi-staff setting, will be critical for success in this role.

The PORTICO Care Centre Intake Coordinator will be directly supervised by the Associate Pastor, SERVE, which includes but is not limited to: leadership support, goal setting, performance evaluation, mentoring and training.

KEY AREAS OF RESPONSIBILITY

Client Intake & Assessment:

Responsible for being a professional and neutral “first point of contact” with clients.
Conduct initial client interviews with intake team of volunteers to assess/discern their needs in mental, physical, financial, or spiritual health.
Be comfortable asking questions to seek to understand client care needs, and ensuring clients meet eligibility criteria.
Collect and maintain accurate records of client information and documentation. o Being cognizant of any conflict of interests which may need to be taken into consideration.

Referrals & Coordination:

Facilitate referrals to appropriate pathways, including care providers or church ministries based on client needs and intake assessment.
Communicate with care providers, volunteers and staff to ensure smooth transitions and proper follow-ups.
Maintain knowledge of PORTICO Care Centre’s services, wellness seminars, and community resources to inform clients.

Data Management:

Enter client information into the appropriate platform, ensuring confidentiality and accuracy.
Follow up as needed to ensure care continuity.

Collaboration:

Work closely with other PORTICO Care Centre volunteers and staff to ensure holistic care for each individual.
Participate in regular meetings with the Associate Pastor, SERVE to discuss client needs and service availability and provide any other updates relating to the PORTICO Care Centre.

Other Duties:

Support administrative tasks as required by the Associate Pastor, SERVE relating to the PORTICO Care Centre.
Follow duty to report requirements as necessary.

LEADERSHIP EXPECTATIONS

As a member of PORTICO’s central support staff, this role requires that you personally embrace, actively promote and support PORTICO’s mission, vision, core values and Ends within all expressions of the church community.

This position is often involved in sensitive situations and the individual may become aware of privileged information requiring sound judgment and discretion. Strict confidentiality must be maintained when dealing with the elder board, pastoral or congregational-related issues. A violation of confidentially is grounds for immediate dismissal.

LEADERSHIP COMPETENCIES AND QUALIFICATIONS

Coursework, certificate or diploma in social work, psychology, counseling, community services, or a related field is an asset.
Prior experience in client intake, assessment, or case management is an asset.
Proficiency in interviewing techniques and comfort in asking questions to assess client needs effectively.
Knowledge of data entry systems or client management software.
Excellent communication and interpersonal skills.
Strong organizational skills and attention to detail.
Ability to handle sensitive information with confidentiality and professionalism.
Empathy, patience, and a passion for helping people, while stewarding resources effectively.
Familiarity with community resources and care systems is an asset.
Work collaboratively as part of a multi-disciplinary team.
Highly adaptable and able to work in a fast-paced environment
